HealthpointServicesTranquil Home Care offers a wide variety of home medical and support care services to meet the unique needs of each individual patient. All of our programs are coordinated by highly trained, experienced and licensed home care nurses whose focus is providing the most appropriate, professional and compassionate care for our patients in a home care setting. We approach all patients as individuals, carefully assessing all medical conditions and needs before assigning a caregiver. Upon completion, we develop an individualized plan of care and assign a professional that best matches the needs of the client.

Alzheimers/Dementia Care
Our caregivers have the experience and training needed to provide appropriate, safe and individualized care in keeping with the distinct challenges often presented by patients living with memory impairments and associated behavioral issues. Our Alzheimer and Dementia home care program has been designed to maintain overall independence and quality of life in a loving and familiar environment.
Companion Care
Companion Care provides emotional support and companionship for seniors who are generally healthy and want to remain independent in their home. It can also include a range of non-medical services, including grooming, transportation, medication reminders, meal preparation, light housekeeping, laundry, groceries, errands and support that help make a senior’s life more manageable.

We also offer ‘off the clock’ volunteer or assistance services for errands or needs that might arise while off the clock to the best of our ability. We are not just an agency thats ‘on the clock’. Caring is our passion, and we are just a call away anytime you need us.

Hospital Discharge/Transfer Assistance
A smooth transition from the hospital to home, a rehabilitation facility, or a nursing home, can dramatically improve the outcome for patients as they move to the next level of care. Our team will help with the planning and implementation of the transfer, as well as the support necessary for the patient and family to ensure continuing recovery and improvement. After a hospitalization or surgery, care may be needed on a temporary basis during recovery. Our staff will help navigate through rehab and assist with bathing, grooming, toileting, wheelchair transfers, medication reminders, meal prep and eating assistance.
Personal Home Care
Our compassionate, trustworthy and certified Home Health Aids and Personal Care Attendants are available to assist with dressing, eating, grooming, medication reminders and light housekeeping. They are trained in all areas of personal care needs, are CPR certified and have met all requirements for employment, including background checks. They work hand in hand with our nurses and therapists to ensure that each patient’s care plan is carried out according to the wishes of the patient, their family and the patient’s physician. Our personal home care team is here to give you peace of mind, knowing that your loved one is in good hands.
Private Duty Nursing
Our caring, professional staff offers you or your loved one customized assistance and hands-on care, twenty-four hours a day. We provide a full spectrum of private care in the client’s private home, assisted or long term care facilities, rehab centers and hospitals.

Our staff will also assist with the personal care services needed to maintain independence in the home, including meal preparation, personal grooming, laundry, light housekeeping, medication reminders, companionship and caring for bed-bound clients. When one-on-one care is needed, we will be there for you, whether it’s an hour a day or for overnight stays.

Respite Care
Caring for an elderly, chronically ill, or disabled family member may simply be a part of daily life, but it is a demanding role and no one is equipped to do it alone. It’s normal to need a break, that can benefit both the patient as well as the caregiver.

Our Respite Care Service provides short-term breaks that can relieve stress, restore energy, and promote balance in your life. Respite care can be for an ill or special needs adult or child, such as a child with autism or an adult with Alzheimer’s. Our services may be arranged for a weekend, a day, or even for an hour. Call us today and we will schedule respite care for you on a regular basis or just one time to give you the break you need.
